Please upload your completed forms at least 24 hours prior to your scheduled appointment.  Otherwise, please bring your completed forms with you to your appointment.  Please do not submit appointment or prescription refill requests here.  Instead, use the forms available on our Contact Us page.

    December 2024 Announcements
    For Current Patients

     We are transitioning to a new billing system and process.  As a result of this changeover, we no longer have access to any credit cards that were on file.  If you receive an invoice by e-mail, you may pay directly from that invoice and the card information will be stored for future visits.  If you have not yet paid an invoice prior to your next visit, we will enter your credit card into the system at that time.  Thank you for your understanding.
     
    Dr. Hwang will be out of the office and covered by Dr. Sue Kanter on Friday, December 13.   You may reach Dr. Kanter through Tanya during regular business hours.  You may reach Dr. Kanter for urgent matters after hours by calling our office telephone number.
     
    In mid-December, we will email invoices for the 2025 Annual Administrative Fee.  We plan to transition from the Tebra (formerly Kareo) billing system to TPCPay, so your invoices will come from TPCPay.  Because we will not automatically have your credit card information in the TPCPay system, please use the link on the invoice to complete payment by January 20, 2025.

    Tanya Alston has been working with Dr. Hwang as a medical assistant for much of Dr. Hwang’s medical career. Patients appreciate her warm personality, professionalism, patient-centered focus and adept technical skills.
